COPD (Chronic Obstructive Pulmonary Disease) Management Program

The goal of the COPD Management Program is to improve the health status for members with COPD. We take a multi-disciplinary, population-based approach that promotes evidence-based diagnosis and treatment. Our goal is to manage health care costs, while helping you achieve optimal control of your disease.

Program interventions assist you in understanding COPD, promoting self-management skills and facilitating a collaborative relationship that can reduce COPD complications.

All members with a diagnosis of COPD are eligible to join the program. Members can be identified for eligibility through:

  • In-patient hospital claims
  • Pharmacy claims
  • Members can be referred to the program by their health care providers
  • Members can enroll themselves in the program

  • Personalized educational material regarding COPD
  • Free, telephonic health coaching by our staff of registered nurses
  • Newsletter articles
  • Web-based program information and self-management tools
  • On-line enrollment and "opt-out options"
