Manage your health

Take control of your health, and treat your conditions by enrolling in our Case and Disease Management program. We are dedicated to ensuring that members receive all the necessary health services needed to live a healthy life. This voluntary program is a collaborative effort to assess, plan, coordinate, monitor and evaluate healthcare options and services that fit your needs.


Who is eligible?

All members who have been diagnosed with conditions that require certain chronic care needs are eligible. This can be verified through:

  • Out-patient medical claims
  • In-patient hospital claims
  • Pharmacy claims
  • Referral by health care providers

You may be referred by your physician or contacted directly by one of our case managers for enrollment in our program.

How does it work? 

Case and disease management is tailored to the unique needs of each member, giving you a voice in the delivery of your care. Our trained professionals help you navigate the health care system through a lifestyle-oriented approach and education. 

As part of the program, an experienced case manager will work with your physician and other members of your health team to offer the benefits you need to receive proper care. Through the use of service plan benefits and community-based resources, your case manager will provide a diverse, innovative and integrated management of care.

  • Outreach associates 
  • Behavioral health specialists

You can self-register online and shortly after submitting your online application, you will hear from a specialist ready to facilitate your treatment process.
